USD 113 and Nemaha Co. Community Health Services will be providing Flu Vaccines this year.

Parent/Guardian needs to:

  1. Complete and sign the consent form.  Print both pages of the consent form and attach.  This form is also available online at www.usd113.org, in the school offices, or at NCCHS
  2. Select Method of Payment:
  • Send a copy of both sides of your student’s health insurance card (BCBS, Meritain, Cigna, Century Health, Coventry, First Health Network, United Healthcare, Aetna, CoreSource, KanCare only) if immunizations are a covered expense.  Insurance will be billed $35 for flu shot.  Parents will be billed for any remaining amount that is not covered by insurance. 

OR

  • Send a $35 payment.  Checks payable to NCCHS.            

Some students may be eligible for flu vaccine at a reduced cost through the Vaccines For Children Program.  Please call NCCHS for information about this program if interested. 

 

Students will not be administered a flu shot unless consent form is fully completed including parent signature with insurance card or payment attached.
