Nemaha County Community Health Services will be offering flu shots to students and staff
NCCHS will be administering flu shots at the following schools on the dates and times below:
Tuesday, October 8 @ SHS (8:30) and SMS (10:00)
Friday, October 11 @ SES (7:30-9:00)
Tuesday, October 15 @ Wetmore (9:00)
Parent/Guardian needs to:
Complete and sign the consent form. Print both pages of the consent form and attach.
Select Method of Payment:
Send a copy of both sides of your student’s health insurance card (BCBS, Aetna, Meritain, Cigna, Century Health, Coventry, CoreSource, First Health Network, United Healthcare, KanCare only) if immunizations are a covered expense. Insurance will be billed $35 for flu shot. Parents will be billed for any remaining amount that is not covered by insurance.
- Send a $35 payment. Checks payable to NCCHS.
Some students may be eligible for flu vaccine at a reduced cost through the Vaccines For Children Program. Please call NCCHS for information about this program if interested.
Students will not be administered a flu shot unless consent form is fully completed including parent signature with insurance card or payment attached.
The flu vaccination forms are attached along with a copy of the instructions.
